Portal vein cavernous transformation refers to the partial or complete chronic obstruction of the portal vein or its intrahepatic branches, leading to impaired portal blood flow and increased portal pressure. Compensatory collateral microvessels form around the portal vein to alleviate the obstruction. Pathological examination of these vessels reveals newly formed collateral vessels resembling cavernous hemangiomas, hence the name portal vein cavernous transformation. This condition is rare, and its prevalence in the general population is currently unknown. The incidence in liver cancer patients ranges from 10% to 40%, and individuals who have undergone splenectomy are at a higher risk of developing this condition. Patients without portal hypertension may be asymptomatic. Secondary portal vein cavernous transformation primarily presents with symptoms of the underlying disease, typically including hematemesis, melena, and splenomegaly.
